We thank you for moving your checking account to Affinity Bank. We think you’ll find our new way of banking a refreshing change from the big banks out there.

Our Affinity Bank Switch For the Better Kit will provide you all the forms you need to make the move to Affinity Bank an easy one. Before you use these forms, you’ll first need to open up your Affinity Bank account.

Once your account is opened, complete the forms in this kit. The forms the kit contains allows you to:

transfer the funds you currently have in your old bank into your new Affinity Bank checking account;
shift any direct deposits you have going into the old account into your new Affinity Bank account;
redirect any automatic payments you currently have coming out of your old account to start being debited from your Affinity Bank account.

Form #1: Authorization to Transfer Funds

Complete this form and fax, send or give directly to your old bank. This will give them the instructions to take the money out of your old checking account so you can deposit it into your new Affinity Bank account. Your bank may be able to do this electronically, provided you can give them your new Affinity Bank account number. Be sure to leave enough funds in the old account to cover any outstanding checks written against the account.

Form #2: Direct Deposit Request

This form is completed and given to your employer or others who send money electronically to you, advising them to redirect your payments to your new Affinity Bank account. If you are redirecting social security, you will want to access a form from the U.S. Social Security Administration website (www.ssa.gov/deposit). If you are redirecting a pension, you can find the forms on the Pension Benefit Guaranty Corporation website (www.pbgc.gov/workers-retirees).

Form #3: Electronic Payment Charge Request

Copies of this form are to be completed and sent to all companies to which you currently have an automatic payment made. You will need to contact each one to determine how to get this form to them – most will accept faxes. Review your old account statements to identify these types of auto pay arrangements –the entries on your statement may include a phone number. You’ll also want to remember to use your new Affinity Bank account number if you go to your creditor’s website to pay their bill.

That’s it. Once you have completed these tasks, you will have successfully switched your account to Affinity Bank.
